Date: April 13, 2010 Objectives: 1. To introduce life haven independent living center. 2. To know the general condition of the visited person with disability. 3. To know what help/assistance life haven independent living center can extend to the visited person with disability. 4. To encourage the visited person with disability to join the activities of life haven independent living center. Venue: (Neddie Malibiran) # 2096 La Mesa St. Ugong, Valenzuela City Participants: Jun Bernandino & Meanne Dao Outcome: All the objectives were met. The center also provided Neddie walker to help her on her mobility. Evaluation: At first, Neddie was a little bit shy. She expressed happiness to have us as visitors. She expressed interest to join us in our coming activities. However, Neddie was unable to join many of the activities that the center had organized. It appears that Neddie has many concerns in her family that she needs to take care. |
