Date:September 1, 2010 Objectives: 1. To introduce life haven independent living center. 2. To know the general condition of the visited person with disability. 3. To know what help/assistance life haven independent living center can extend to the visited person with disability. 4. To encourage the visited person with disability to join the activities of life haven independent living center. Venue: (Cristobal Estigoy) life haven independent living center (Cristobal decided to visit the office instead of us visiting him.) Participants: LHILC staffs w/ Arnel Aromyn Outcome: 1. LHILC staffs successfully introduced the center and its services to Cristobal. 2. LHILC staffs learned the general condition of Cristobal and his need for a wheelchair. 3. It is very apparent that Cristobal is interested to join in all the activities of LHILC. Evaluation: At first, Cristobal is hesitant to entertain LHILC staff on the phone conversation. For some reason, he decided to visit the center. We observed his enthusiasm while we were sharing the independent living philosophy. We think Cristobal will be a good leader in the future. |
