Date: September 1, 2010 Objectives: 1. To introduce life haven independent living center. 2. To know the general condition of the visited person with disability. 3. To know what help/assistance life haven independent living center can extend to the visited person with disability. 4. To encourage the visited person with disability to join the activities of life haven independent living center. Venue: (Gulliver Jhon Garduque)# 11a Lower Tamaraw Marulas, Valenzuela City Participants: Jun Bernandino, Eric Dy & Cris Estigoy Outcome: LHILC staffs successfully introduce the center. He is also willing to attend the activities of the center depending on his availability. He is still undergoing therapy. Also, his mother is not supporting the idea of his son joining the activities of the center. Evaluation: Gulliver gladly welcomed the staffs. He requested to be visited more often. He wanted to go out and enjoy social life together with the staffs of the center. |
