Date: September 1, 2010 Objectives: 1. To introduce life haven independent living center. 2. To know the general condition of the visited person with disability. 3. To know what help/assistance life haven independent living center can extend to the visited person with disability. 4. To encourage the visited person with disability to join the activities of life haven independent living center. Venue: (Lucia Gueco) # 26 Jasmin St. Happy Site Subd. Marulas Valenzuela City Participants: Jun Bernandino, Eric Dy, Cris Estigoy & Manny Payumo Outcome: 1. LHILC staffs successfully introduced the center and its services to Lucia. 2. LHILC staffs learned the general condition of Lucia. 3. It is very evident that Lucia is interested to join in all the activities of LHILC. Evaluation: Lucia showed interest. She compared what we were presenting to what she already knows. She said independent living is different and promising. |
