To continue encouraging the visited person with disability to join the activities of life haven independent living center. Venue: (Ailyn Bermas) # 2425 St. Matthew St. Gen. T. De Leon Valenzuela City Participants: Jun Bernandino & Eric Dy (2nd) Outcome: LHILC staffs follow up on her about her situation. Ailyn open up again that she need a powered wheelchair for her plans to become more productive to help in the household income. The staffs explained to Ailyn about the responsibilities and how to maintain a powered wheelchair. Evaluation: Ailyn wanted to have a powered wheelchair to help her husband earn additional income for their family. It can also help her go to places without seeking much assistance from others. She also promise to take good care of the wheelchair once she have it. |
