De Leon Valenzuela City Participants: Eric Dy & Teody Auxilian (1st) Outcome: 1. Life haven independent living (LHILC) staff successfully introduced LHILC and its services to Ailyn and her family. 2. LHILC staffs learned the general condition of Ailyn and her family and her need for a powered wheelchair. Your evaluation: Ailyn is very happy when visited by LHILC staffs. She shows interest in participating in LHILC group activities. |
