Date: March 10, 2010 Objectives: 1. To introduce life haven independent living center. 2. To know the general condition of the visited person with disability. 3. To know what help/assistance life haven independent living center can extend to the visited person with disability. 4. To encourage the visited person with disability to join the activities of life haven independent living center. Venue: (Cenen Leonardo) # 3029 F. Bautista St. Ugong, Valenzuela City Participants: Abner manlapaz & Meanne Dao Outcome: Abner and Meanne shared the IL Philosophy. The LHILC and its services were introduced to Cenen. The needs of Cenen for personal assistance and wheelchair were identified. The center provided Cenen wheelchair for his mobility. Now, he can go out of their home and join the activities in their neighborhood. Evaluation: Cenen showed interest. Unfortunately, Cenen is not consistent in joining the center's activities. When Cenen was asked for reasons why he misses some of the activities, he said that he needs to ask permission from his mother whenever there are activities that we want to invite him. However, his mother is telling a different information. According to Cenen's mother, Cenen is not reliable about anything and apologizes for Cenen's attitude. Whoever is telling the truth will remain to be seen in the future. What is important is to continue reaching out to Cenen and provide alternatives to enable Cenen so he could join the activities of the center without relying on his mother. At the moment, it is Cenen's mother who always accompany him whenever he joins the activities of the center. |
