Date: April 17, 2010 Objectives: 1. To introduce life haven independent living center. 2. To know the general condition of the visited person with disability. 3. To know what help/assistance life haven independent living center can extend to the visited person with disability. 4. To encourage the visited person with disability to join the activities of life haven independent living center. Venue: (Jayrine Marie Paguio) #65 San Francisco St. Karuhatan Valenzuela Participants: Eric Dy & Teody Auxilian Outcome: 1. LHILC staffs successfully introduced the center and its services to Jayrine's family. 2. LHILC staffs learned the general condition of Jayrine and her family and her need for a wheelchair. 3. The center provide Jayrine a new wheelchair. Now, her parents no longer need to carry Jayrine whenever they need to move her from one place to another. Evaluation: The family shows interest to participate in LHILC activities and willingness to attend support group activities. So far, Jayrine's mother has attended all the center's activities. Her activeness is very encouraging to the group. |
