Date:August 21, 2010 Objectives: 1. To introduce life haven independent living center. 2. To know the general condition of the visited person with disability. 3. To know what help/assistance life haven independent living center can extend to the visited person with disability. 4. To encourage the visited person with disability to join the activities of life haven independent living center. Venue: (Marilyn Kris Padrinao) # 93 Padrinao St. Karuhatan Valenzuela City Participants: Jun Bernandino & Eric Dy Outcome: 1. LHILC staffs introduced the center and its services to Marilyn's family. 2. LHILC staffs learned the general condition of Marilyn and her family and her need for a wheelchair. Marilyn has intellectual and mobility disability. Evaluation: LHILC staffs believes that Marilyn Kris needs personal assistance and wheelchair. We think that there is a need to make the family members understand that Marilyn Kris can enjoy being around with other people and it would be dispiriting on Marilyn's part to be always in the room alone. |
